Case Management is available after participating in a Just The Facts workshop.  Case Managers can help formerly incarcerated people and others in need with housing opportunities, transportation, legal services, medical needs, and more.


Who Can Qualify

Anyone who wants to better themselves can qualify for Healthy You, Inc. case management, including:

  • Currently incarcerated
  • Formerly incarcerated
  • Veterans
  • Anyone looking for affordable housing
  • A single parent with no support
  • Families looking for a better future
  • Anyone wanting to get their GED
  • High school students who see no future

Upon release, Healthy You, Inc. case managers can assist with a variety of things to help you make a positive transition into your community, including:

How To Qualify

Healthy You, Inc. has an interest in YOU beyond the classroom. We want to help you achieve life-long sustainability.

To qualify for case management services, one must complete a Healthy You, Inc.– Just the Facts Life Skills workshop. This workshop covers a variety of topics including positive conflict resolution, identifying communication styles, maintaining healthy relationships, understanding and managing money, and defining and changing destructive behaviors.

An incarcerated individual may begin receiving case management services when he or she is 18 months or less to end of sentence.

Goals of Case Management

Healthy You, Inc. never wants you to feel hopeless. Every person has value, regardless of his or her station in life.

Case management can help you meet several goals:

  • Family reunification
  • Reduced likelihood of recidivism
  • Increased self esteem
  • Positive return to the community

Your case manager will partner with you to provide a complete assessment. You will then be guided through a development of goals, provided with relationship building information and help to develop a home plan for your return to the community.

*Available if within 18 months of end of sentence. Most services are available at no charge.
